Thresholds and small changes in floor level between rooms can be lowered, levelled or replaced. New flooring is on the City of Ottawa's list of projects that do not need a building permit.
You may see a 13 mm figure quoted. In the Building Code it belongs to the barrier-free section for public buildings, where a change in level up to 13 mm must be bevelled at no more than 1 in 2. It is a useful reference, but it is not a rule for your house.
Outside, the Code requires the finish on ramps and landings to be slip-resistant, wear-resistant and smooth. At an entrance door, a ramp or landing may be needed where the step is too high for a threshold change alone; see entrance ramps.
Funding to check: the March of Dimes program lists anti-slip flooring (vinyl or ceramic tiles only). The Home Accessibility Tax Credit covers renovations that reduce the risk of harm in the home or help an eligible person move around it. Each program decides eligibility.
